- Slice five onions thinly. Heat olive oil in a skillet over medium heat. Add onions and sprinkle with sugar. Stir and caramelize for 30-40 minutes.
- In the skillet with onions, melt butter. Stir in flour to form a roux, gradually add heavy cream while whisking until thick. Mix in caramelized onions and Parmesan.
- Boil water in a large pot, add salt and cavatappi macaroni. Cook until al dente, about 7-8 minutes. Drain and return to pot.
- Pour béchamel sauce over macaroni and mix until well-coated.
- Preheat oven to 350°F (175°C). Spread pasta mixture into a baking dish, top with remaining onions and breadcrumbs. Bake for 10-15 minutes until golden.
- Remove from oven, let sit, garnish with parsley and serve hot.
